Stony Brook Southampton Hospital has partnered with Verissima to provide an online method for requesting medical records. To access the request form, visit southampton.stonybrookmedicine.edu, click on the "Patient and Visitors" tab, then select "How to Request Medical Records." Scroll to the red button to access the online forms. If you are requesting your own records, choose the "Patient Request Form." If requesting someone else’s records and you have the authority, select the "Representative Request Form."
