The following is a compilation of state laws that AVMA research has identified governing the retention of veterinary records. Typically states require veterinarians to keep records anywhere from 3-5 years after the last patient exam or treatment.
(1) Name and address of client and animal. (2) Age, sex, breed, species, and color of the animal. (3) A history or pertinent information as it pertains to each animals medical status. (4) Data, including that obtained by instrumentation, from the physical examination.
Access to Clinical Records While recognizing the practitioners ownership of the clinical records, the case judgment states that the information in those records belongs to the patient and that the patient has a right to access that information upon request.

7 Types of Records Your Veterinary Practice Should be Keeping Tax records. First, consider the records you need to substantiate your annual income tax return. Accounting records. Travel and entertainment expenses. IRS audits. Employment tax records. Corporate records. Computer recordkeeping.
Room Cards, Animal Census, and Daily Records An animal census/inventory for the room, environmental chamber, pen or tank must be maintained. This should be a running count that is updated as necessary. A daily activity record is also required.
In a solo practice, the issues are more straightforward: the physician is the custodian of the medical record and therefore has control over access to the record, as well as its retention and disposal.
