If you want to add an image to your survey you can use the green bar (Change Question Type) at the top right of your page to insert a graphic. You can also insert a graphic in the text of a question by using the Rich Content Editor and clicking on the graphic icon (top row, second from left).
You can add images by uploading them from your computer. To add an independent image to a survey: Click the Design Survey tab. In the Build section of the left sidebar, drag and drop Image onto your survey.

Adding a Logo At the top of the survey, click + Logo. Upload a logo from your computer. Adjust the logo settings in the left sidebar: Logo Size - Choose to display your logo at its Actual Size, Small (100px), Medium (200px), Large (300px).
Adding Images to Question Text Add your question or image type by dragging and dropping it from the Build section of the left side bar onto your survey. From the rich text editor, click Insert or Edit Image to add an image from your computer. The image format must be . jpg, .
Click + New Page below any page in your survey to add a new page in that spot. Adds an intro page to the beginning of a survey. Creates a new page in your survey. You can either click to add the page to the end of the survey or drag and drop the page after another page in the survey.
