The S3 and S4 heart sounds are both low pitch heart sounds heard in diastole. They differ when they are heard in diastole, S3 being heard early in diastole while S4 is heard later in diastole.
The summation gallop is a loud gallop that occurs in patients with tachycardia. In fast heart rhythms, diastole shortens, causing the events that produce S3 (rapid early diastolic filling) to coincide with those producing S4 (atrial systole). The resulting sound sometimes is louder than the patients S1 or S2.
The S3 heart sound is a low-pitched sound that doctors can hear when blood rushes rapidly from the hearts atrium into the ventricle. Sometimes, particularly in children and athletes, it is a typical sound. However, in other cases, it may also indicate that an individual has congestive heart failure.
After age 40, a third heart sound is usually abnormal and correlates with dysfunction or volume overload of the ventricles. Any cause of ventricular dysfunction, including ischemic heart disease, dilated or hypertrophic cardiomyopathy, myocarditis, cor pulmonale, or acute valvular regurgitation, may qualify.
What is an S3 Heart Sound? The third heart sound is a short, low frequency sound heard best with the stethoscopes bell. It occurs early in diastole and is dull in quality. As an extra heart sound, it creates a galloping rhythm with a cadence like word Kentucky.
The sound occurs just after atrial contraction at the end of diastole and immediately before S1, producing a rhythm sometimes referred to as the Tennessee gallop where S4 represents the Ten- syllable.
It occurs at the beginning of the middle third of diastole, approximately 0.12 to 0.18 seconds after S2. This produces a rhythm classically compared to the cadence of the word Kentucky with the final syllable (-CKY) representing S3.
A S3 can be a normal finding in children, pregnant females and well-trained athletes; however, a S4 heart sound is almost always abnormal. CLINICAL PEARL: A S3 heart sound is often a sign of systolic heart failure, however it may sometimes be a normal finding.
