ACL surgery can fail, even in the best of circumstances. It is generally felt that a well-done ACL reconstruction has about a 5% chance of failure due to trauma. The most common cause of an ACL graft failure is a technical error with malposition of the original ACL reconstruction tunnels.
The risk of re-tearing the ACL is 5%, but with many exceptions. There are several ways to limit the possibility of re-injury: Follow all recommendations provided by our orthopedic surgeon post surgery. Carefully adhere to the physical therapy rehab protocols.

Investigators have reported reinjury rates following ACL surgery to range as high as 30-39%. This rate of reinjury results in as many as 4 in every 10 surgeries being attributed to reinjury. This level of reinjury following ACL surgery is unacceptable.
After an ACL reconstruction, up to 14% of patients go on to suffer a graft rupture, an ACL re-rupture [8]. Several risk factors for suffering an ACL re-rupture have been identified, including younger age, high activity level, female sex, and the use of allograft [9,10,11].
How many times can an ACL be repaired? There is no real limit to the amount of times the ACL can be reconstructed. However, each successive surgery may become technically challenging due to bone loss. This requires that your surgeon have expertise in complex revision ACL surgery.
Between 6% and 22% of all individuals after ACL injury will sustain a second ACL injury (19, 20). Both younger age and returning to sport are risk factors of second ACL injury, with a second ACL injury rate as high as 25% to 34% in young athletes who have returned to sport (3) (4)(5).
