Very minor tears (sprains) may heal with non-surgical treatments and regenerative medicine therapy. But full ACL tears cannot be healed without surgery. If your activities do not involve making pivoting movements on the knee, physical therapy rehabilitation may be all you need.
ACL reconstruction is typically an outpatient procedure. The surgeon will remove the damaged ligament, then replace it with a new oneeither an autograft (tissue taken from another part of the body, such as the kneecap tendons or hamstring) or an allograft (tissue from a deceased donor).
What Is ACL Reconstruction? A torn ACL usually is treated with a procedure called an ACL reconstruction. Surgeons replace the damaged ligament with new ACL graft tissue either taken from the patients own body (tissue from the main patellar tendon or the hamstring) or donated from someone else (called an allograft). A revision ACL reconstruction is a second surgery needed to repair a torn anterior cruciate ligament. ACL revision surgery is a more challenging operation for the orthopedic surgeon than a primary reconstruction. There are a number of surgical options for reconstructing the ACL. The type of procedure done may vary from patient to patient depending on a specific surgeons preference as well as factors unique to an individual patient. The surgical procedure is most commonly performed using arthroscopic techniques.
In primary repair of a torn ACL, the surgeon places stitches in the injured portion of the ACL and uses an anchor to reattach it back to the bone where it tore away. Sometimes an additional stitch is added as an internal brace to protect the healing ligament. Reconstruction is often recommended because most injured ACLs cannot be stitched back together (repaired) due to the surrounding environment. Recent efforts have been made to find a way to improve this environment in order to repair the ACL without using a graft.
