The ACL arises from the anteromedial aspect of the intercondylar area on the tibial plateau and passes upwards and backwards to attach to the posteromedial aspect of the lateral femoral condyle. The ACL tibial footprint substantially overlaps the anterior root lateral meniscus footprint 6.
There are a number of surgical options for reconstructing the ACL. The type of procedure done may vary from patient to patient depending on a specific surgeons preference as well as factors unique to an individual patient. ACL injuries dont always need surgery. When people have minor ACL injuries, doctors may suggest a nonsurgical ACL injury treatment. In primary repair of a torn ACL, the surgeon places stitches in the injured portion of the ACL and uses an anchor to reattach it back to the bone where it tore away. Sometimes an additional stitch is added as an internal brace to protect the healing ligament.
If a repaired ACL fails, it can only be revised with an ACL reconstruction. Having to redo any kind of ACL surgery results in higher rates of failure, lower rates of successful return to sports activity, and increased risk of developing osteoarthritis in the knee.
The decision to proceed with a second ACL surgery depends on the patient, the condition and stability of his or her knee, the desired activity level and imaging findings. Patients are advised to seek out a specialist with ample experience in revision ACL surgery for the best chance of a good outcome.
