Sometimes a revised manuscript goes out to a new reviewer, so although the minor revisions may have been done in a thorough manner, the new may identify other issues with the manuscript, which might result in a recommendation to reject.
It depends on journal and reviewers . Usually 20 to 30 days minimum required.
Revise and resubmit, somewhat less likely (2030%). Its fortunately rare, but I have been asked for revisions and then even when theyve been done (at least to the authors satisfaction), its been subsequently rejected, usually because points other than those in the original review have arisen. Grrrr.
Revise and resubmit means exactly what it says: if you revise the manuscript and resubmit it, we will look at it again (hopefully with the same editor and reviewers, but not necessarily). Typically this means the required revisions are substantial enough that it will go to reviewers again.
Just as Revise and Resubmit is not a rejection, revising and resubmitting your manuscript isnt a guarantee of acceptance. Some consider a Revise/Resubmit decision as a tacit acceptance, but really there are no promises here, particularly if you are called upon to make major revisions to your manuscript.
Major revisions are almost always sent back to reviewers for a re-review to determine whether the comments have been addressed and no new comments are raised. If authors are unable to satisfactorily address the comments, further revisions may be required or the paper may be rejected.

They found that 90% of articles needing minor revision and about 50% of those needing major revision were eventually accepted.
So, what does a journals editorial decision of major revisions mean? In short, major revisions means youve got work to do on your manuscript, but youre still very much in the game. You had a great research idea to start with. Youre not far from getting it accepted and published.
