ACL tears and ACL surgery fortunately most of the time works out quite well however in a number of cases the ACL reconstruction or repair fails and in that setting we can rebuild the ACL using stronger donor tissue now from the quadriceps tendon and we can understand their reasons for the failure which most commonly are either a misplacement of the first ligament or failure to understand the other ligaments and other tissues around the knee that are also damaged and so today whenever we do a revision reconstruction or rebuild a knee that has had a previous failed surgery were almost always combining it with a whats called a lateral extraarticular reconstruction where we add tissue on the side of the knee to diminish the excess rotation and stabilize the joint to help that new ACL have a more successful outcome so to summarize theres basically two major reasons that we see for ACLS to fail one is misplacement of the first graft two is a misunderstanding of all the injuries that occur

A revision ACL reconstruction is a second surgery needed to repair a torn anterior cruciate ligament. This is a more challenging operation for the orthopedic surgeon.
ACL reconstruction surgery has a success rate of 80-90%.
The procedure usually takes less than two hours. It will require an incision to remove a tendon if you are undergoing an autograft, in which a tendon from another part of your body is inserted in your knee.
There is no real limit to the amount of times the ACL can be reconstructed. However, each successive surgery may become technically challenging due to bone loss. This requires that your surgeon have expertise in complex revision ACL surgery.
How long is the recovery after revision ACL reconstruction? In general, the recovery is the same as primary ACL reconstruction: 6 months. Bracing is used for 6 weeks after surgery. Physical therapy starts one week following surgery and continues for approximately 3-4 months following surgery.

Because ACL reconstruction is a surgical procedure, it carries certain risks, including: bleeding and blood clots. continued knee pain. disease transmission if the graft comes from a cadaver. infection. knee stiffness or weakness. loss of range of motion. improper healing if the graft is rejected by your immune system.
In general, most revision ACL reconstructions take 9-12 months to adequately heal and for the athlete to recover their strength, endurance, proprioception and balance.
