[Applause] [Music] [Applause] [Music] so this is a picture of london i took back in 1976 when the sun last shown here now two-stage revision acl is really unattractive to the patient its usually three to six months between the two stages and if youre a professional athlete that may actually mean that you lose another season for that for them that could be career ending its more expensive theres more morbidity and i suspect a lot of surgeons do two-stage procedures theres a reflex decision they dont really consider whether a one stage is possible now of course the truth is i do do two-stage procedures ive even published on it in this example on the right you can see a huge defect in the tibia the only answer here is a bone grafting first stage and a second stage revision but the truth is i do very very few two stage procedures if you look at my series of 204 patients in that um time period from 0 9 17 only three of them were two stage so youve got to realize my practice is real

Because revision ACL reconstruction is a more difficult operation to perform compared to primary ACL surgery, patients should choose an orthopedic surgeon with ample experience and with whom they feel comfortable. The doctor should take the time to answer all of a patients questions in nontechnical terms.
Types of ACL Surgery Autograft. Your doctor uses a tendon from somewhere else in your body (like your other knee, hamstring, or thigh). Allograft. This type of graft uses tissue from someone else (a deceased donor). Synthetic graft. This is when artificial materials replace the tendon.
The gold standard in ACL injury evaluation is diagnostic arthroscopy3,4; however, the diagnostic accuracy of clinical diagnostic tests and magnetic resonance imaging (MRI) is debatable. Because of the frequency of knee injury,3 MRI is a common diagnostic tool used for imaging and diagnosis.
ACL LAXITY TESTS The most frequently used tests for assessing ACL laxity are the anterior drawer test, Lachman test, Maclntosh test, jerk test, flexion rotation drawer test, Slocum test, and Losee test.
ACL reconstruction surgery has a success rate of 80-90%.
Two-stage revision ACLR typically involves an initial bone-graft procedureto fill the widened or misplaced tunnelsand subsequent time to allow for the bone graft to heal sufficiently before the second stage is undertaken [5]. A relatively small but challenging subset of patients requires two-stage revision ACLR.
You are most at risk of tearing your ACL graft when you return to sport following your ACL reconstruction. Returning to sport within 9 months of ACL surgery puts you at a higher chance of tearing your ACL graft.
What is a revision ACL reconstruction? A revision ACL reconstruction is a second surgery needed to repair a torn anterior cruciate ligament. This is a more challenging operation for the orthopedic surgeon.
If you do tear your ACL and have surgery or the Perc ACLR procedure there is a chance of re-tear. Those tears can again be treated with the Perc ACL procedure or with another surgery if a larger tear. Some ACL tears and re-tears can be treated just with physical therapy to compensate in a few patients we call copers.
The procedure usually takes less than two hours. It will require an incision to remove a tendon if you are undergoing an autograft, in which a tendon from another part of your body is inserted in your knee.
