In wide-local excision, the skin cancer and a small margin of healthy tissue around it is cut out, usually in a football-shaped ellipse. Once the tissue has been removed, the edges of the wound are sutured together. The tissue then is sent for processing and margin evaluation by a pathologist.
An adequate clear margin is thought to be more than 3 mm6, more than 5 mm1,7,9, or 7 mm4. ing to most authors, an adequate pathological margin is at least 5 mm.
Abstract. Squamous cell carcinoma is the most common malignant tumor of the hand. Most hand surgery texts recommend wide excision with 1- to 2-cm margins. During a 10-year period, 32 patients with 35 lesions were treated with marginal surgical excision for squamous cell carcinoma of the hand.
In relation to SCC, surgical margins of 4 mm are generally recommended for low-risk lesions and 6 mm for high-risk lesions.
Background: Controversy exists regarding the optimal surgical margin for cutaneous squamous cell carcinoma (cSCC). Current NCCN Guidelines recommend excision with a 46-mm clinical margin for low-risk cSCC and wider ( 6-mm) clinical margin for high-risk cSCC tumors.
Conclusion: Minimal margins of excision of 4 mm around the clinical borders of the squamous cell carcinoma are proposed for all but the high-risk tumors, in which at least a 6 mm margin is recommended.
Whether or not the margins contain cancer cells doesnt usually affect your treatment. However, in rare cases after a mastectomy, the deep margin (the margin closest to the chest wall) contains cancer cells. In these cases, more surgery and/or radiation therapy may be recommended.
The recommended treatment for cutaneous squamous cell cancer (CuSCC) of the head and neck is Mohs surgical excision or wide local excision. Excision is recommended to a gross surgical margin of 46 mm however this is based on limited evidence and specify a goal histologic margin.
