A lumpectomy with clear margins may be the only surgery needed to treat breast cancer. Positive margins, on the other hand, may indicate that all of the cancer was not removed, and another surgery in the same area may be needed to remove more tissue.
Margin width should be 1 cm for melanomas 1 mm thick, 1 or 2 cm for melanomas 1 to 2 mm thick, and 2 cm for melanomas 2 mm thick. The margin width for wide local excision of a melanoma in situ should be 5 mm. Standard wide local excision margin recommendations also apply to melanomas of the skin of the digits.
The recommended treatment for cutaneous squamous cell cancer (CuSCC) of the head and neck is Mohs surgical excision or wide local excision. Excision is recommended to a gross surgical margin of 46 mm however this is based on limited evidence and specify a goal histologic margin.
In relation to SCC, surgical margins of 4 mm are generally recommended for low-risk lesions and 6 mm for high-risk lesions. An adequate surgical margin for BCCs has been widely debated.
A clear, negative, or clean margin means there are no cancer cells at the outer edge of tissue that was removed. A positive margin means that cancer cells come right out to the edge of the removed tissue and have ink on them.
The current NICE guidelines recommend a margin of at least 1cm to people with a diagnosis of stage I melanoma (Breslow depth 1-2mm), and margin of at least 2cm to people with stage II melanoma (Breslow depth 1-2mm). For melanoma in situ, a 0.5cm margin is sufficient.
Excisional biopsy with narrow (2 mm) clinical margins is recommended as the standard approach for the biopsy of clinically concerning pigmented lesions. If this is not possible, the size of the lesion should be communicated to the pathologist.
