More than 200,000 ACL reconstruction surgeries are performed each year in the United States, and 1 percent to 8 percent fail for some reason. Most of those patients then opt to have their knee ligament reconstructed a second time, but the failure rate on those subsequent surgeries is almost 14 percent.
There is no real limit to the amount of times the ACL can be reconstructed. However, each successive surgery may become technically challenging due to bone loss. This requires that your surgeon have expertise in complex revision ACL surgery.
How long is the recovery after revision ACL reconstruction? In general, the recovery is the same as primary ACL reconstruction: 6 months. Bracing is used for 6 weeks after surgery. Physical therapy starts one week following surgery and continues for approximately 3-4 months following surgery.
In general, the success rates in the literature for ACL revision surgery are approximately 75%. A very careful assessment of the cause of the graft failure is necessary to try to maximize an individual patients outcome with an ACL revision surgery.
A revision ACL reconstruction is a second surgery needed to repair a torn anterior cruciate ligament. This is a more challenging operation for the orthopedic surgeon.
The procedure usually takes less than two hours. It will require an incision to remove a tendon if you are undergoing an autograft, in which a tendon from another part of your body is inserted in your knee.
The decision to proceed with a second ACL surgery depends on the patient, the condition and stability of his or her knee, the desired activity level and imaging findings. Patients are advised to seek out a specialist with ample experience in revision ACL surgery for the best chance of a good outcome.
Conclusion. Multiple revision ACL reconstruction surgery appears to have reasonable functional outcomes but is associated with a relatively high failure rate. Activity level following repeat revision surgery is diminished compared to the preinjury state, but most patients are able to return to recreational sports.
