If you and the mother are not married to each other, you can only be listed on the birth certificate if you have signed an Affidavit of Parentage or a court has found that you are the father. You have the right to petition the court to have yourself declared the childs father.
You may obtain a rescission form by calling the Maryland Department of Health, Division of Vital Records at (410) 764-3038; ii. Within 60 days of signing, either party named in the Affidavit appears in court in a proceeding related to the child and informs the court of his or her decision to rescind; or iii.
Birth certificates for individuals with valid, government-issued photo identification who were born in Maryland are available for same day service at the Division of Vital Records in Baltimore located at 6764-B Reisterstown Road, Baltimore, Maryland 21215.
You may obtain a Rescission Form by calling the Maryland Department of Healths Division of Vital Records at 410-764-3182. If it has been more than sixty (60) days since you signed the Affidavit, the Rescission Form will NOT be effective. After the sixty (60) days, the Affidavit can only be nullified by a court order.
A person may change their name for any reason unless it is for illegal purposes or interferes with the rights of others. They must petition a court for this change. A parent may change the name of their child born in Maryland one time within the first year. After that time, they must petition a court for the change.
An Affidavit of Parentage (AOP) is a legal document that allows a parent who gave birth to a child to add the name of the other parent of the child to the birth certificate.
