The surgery requires harvesting of tissue from other parts of the leg which can be painful. Tunnels are drilled within the bones and there is usually postoperative pain and swelling associated with this. Therefore, this would be classified as a major operation.
Many, but not all, patients opt for revision surgery. People who do not experience instability in their knee and do not wish to return to cutting and pivoting sports may decide not to have surgery. If they wish to remain active, they may engage in a different sport.
Definition. Anterior cruciate ligament (ACL) revision is a surgical procedure performed to address a failed or insufficient primary ACL reconstruction. The goal of the revision surgery is to restore knee stability, function, and prevent further damage to the knee joint.
At about two to four months following surgery, patients should be able to begin regaining all of their functional movement patterns. Full strength in the quadriceps should return about nine to twelve months following surgery.
CPT Code: 29888 The surgery is performed arthroscopically. An ACL reconstruction is sometimes referred to, incorrectly, as an ACL repair. A torn anterior cruciate ligament cannot be repaired, and must instead be reconstructed with a tissue graft replacement.
Because revision ACL reconstruction is a more difficult operation to perform compared to primary ACL surgery, patients should choose an orthopedic surgeon with ample experience and with whom they feel comfortable. The doctor should take the time to answer all of a patients questions in nontechnical terms.
The average revision rate following revision ACL reconstruction in athletes is around 13.7% compared with the average revision rate of 2.9%5.8% following primary ACL reconstructions.
A very thorough physical exam is necessary but these secondary studies are often required to choose the best plan of treatment for a revision ACL reconstruction. In general, the success rates in the literature for ACL revision surgery are approximately 75%.
CPT Code 27427 - Repair, Revision, and/or Reconstruction Procedures on the Femur (Thigh Region) and Knee Joint - Codify by AAPC.
In general, most revision ACL reconstructions take 9-12 months to adequately heal and for the athlete to recover their strength, endurance, proprioception and balance.
