There are four commonly used grafts for ACL reconstruction. Patellar tendon autograft. The surgeon removes the middle third of your patellar tendon and pieces of bone (bone plugs) from your shin and kneecap. ... Hamstring tendon autograft. ... Quadriceps tendon autograft. ... Allograft.
In a sense, bone-to-bone healing is similar to fracture healing and is faster and stronger than soft-tissue healing. Although bone grafts have been shown to heal to host bone within 6 weeks, soft-tissue grafts take 8 to 12 weeks to fully incorporate.
Types of ACL Surgery Autograft. Your doctor uses a tendon from somewhere else in your body (like your other knee, hamstring, or thigh). Allograft. This type of graft uses tissue from someone else (a deceased donor). Synthetic graft. This is when artificial materials replace the tendon.
The patellar tendon and hamstring autografts are the most common choices for ACL reconstruction. The patellar tendon runs from the knee cap (patella) to the lower leg bone (tibia). Surgeons have the most experience with this autograft and it is the most widely used.
ACL reconstruction has been around for decades and has been the go-to procedure for torn ligaments. Reconstruction has a high success rate but can take 6-12 months for a full recovery. ACL repair seeks to keep the patient's ligament in place to promote self-healing. The ligament may also require stem cells.
The patellar tendon graft (PTG) has always been the gold standard for anterior cruciate ligament (ACL) reconstruction. Yet, most orthopedic surgeons prefer hamstring grafts for younger athletes and cadaver grafts for older patients.
However, it may take up to a year to return to your pre-injury level of play. Although ACL surgery has a high likelihood of success, there is always the possibility of reinjury. The best method to limit this risk is to adhere to your physician and physical therapist's rehab and return-to-play instructions.
The graft complex is actually at its weakest at around the 6 week post operative mark. Kinematic research has shown that open chain exercises cause significantly more anterior tibial displacement and hence more strain on the graft than closed chain exercises.
Every surgically reconstructed anterior cruciate ligament can retear. The risk ranges from one or two percent to more than 20 percent. The replacement ligament (graft) chosen for your surgery can significantly increase or decrease your chance of a retear.
