I was a caregiver for my brother basically for his entire adult life even before he got sick. My brother could not read and write, so I was his caregiver and I went along with him to his hospital visits, his doctor visits, just to explain to him exactly what was going on and make sure he understood the things he was signing and that sort of thing. He had some liver issues, he was diabetic. And unfortunately we werent aware of Advance Directives. (soft music) Advance Directive is a legal document that shares with everyone what you will like people to know about your wishes. I dont think Ive realized the task at hand, how important it was until he actually did get sick. And unfortunately we didnt have that discussion about what he wanted if he was incapacitated and unable to make decisions for himself. End-of-life documents, or advance directives, help ensure your healthcare wishes are carried out as you near death and after you die. Theyre also used if youre incapacitated, meaning you are unable to tend to matters regarding your own well-being (e.g., in a coma).
Advance directives are legal documents that provide instructions for medical care and only go into effect if you cannot communicate your own wishes. The two most common advance directives for health care are the living will and the durable power of attorney for health care.
Living Will. A will is one of the most common documents you hear about when it comes to end-of-life planning. Last Will and Testament. Living Trust. Letter of Intent. Financial Power of Attorney. Health Care Power of Attorney. Letter of Competency. Organ Donor Card.
I want to spend my last days in the hospital. I want to die at home. I want hospice care. I want you to take all necessary steps to keep me alive (including with a breathing machine, artificial feeding and dialysis).
These include: Living Will. A living will is a written document that specifies what medical treatment you would or would not want in the event you are in a terminal condition or a persistent vegetative state. Power of Attorney. Health Care Instructions.
Communicating your priorities and hopes for your care near the end of life. No one wants to have to think about their future medical needs. But, to be sure that your future healthcare priorities are met, its important to plan ahead.
Advance directives are legal documents that are completed in advance of the need for major medical decisions to specify which healthcare options are or are not desired, at present or in the future, and to designate an individual(s) who will speak on the patients behalf if the patient cannot speak for his or herself.
