Distributions from a 401k that are reported on a Form 1099-R are reported on Lines 16a and 16b of Form 1040. Line 16a reports the total distribution amount and line 16b reports the taxable amount.
What do I need to fill out Form 1040? Social Security numbers for you, your spouse and any dependents. Dates of birth for you, your spouse and any dependents. You report the final amount on Line 30 of your 2021 federal income tax return (Form 1040 or Form 1040-SR). The recovery rebate credit is a "refundable" credit, which means you'll get a tax refund if the credit is larger than the tax that you would otherwise have to pay.
What exactly do I put on Line 30? You will only fill in the amount of the total credit you are due from either the first or second round or both. If you received a partial payment you would include the balance of the credit you are owed.
Line 34 calculates any overpaid amount, subtracting line 24 from line 33.
Luckily, the Internal Revenue Service can provide you with copies of your original tax returns. As long as you filed the original tax return on Form 1040, 1040-SR, 1040A or 1040EZ, you can request a copy by filing Form 4506 with the IRS.
The biggest change to Federal Form 1040 is that a new Line 30 has been added for the Recovery Rebate Credit. This is for taxpayers who didn't receive payments or could have received a larger payment from the government when economic impact payments (stimulus checks) went out in 2021.
