Most DNR forms do not need to be docHubd, although they can be. Typically, two adult witnesses are required to watch you sign the order, and then add their own signatures to attest that you were sound of mind. However, you can replace these two witness signatures by having your DNR Form signed by a notary.
Documentation in the patients medical record of a decision not to do CPR must include both a Physicians Order and an entry in the Progress Notes. Use of the standardized DNR Order Form is preferred, but is not required in cases where the attending physician prefers a narrative method of documentation.
How to Write a DNR Form Write the name of the patient and then state where you or that person lives. Make sure to make the requests known, that CPR, or another treatment, is to be withheld. Acknowledgement: The people who are in charge of the patient must sign the document.
