What are the five rules of incident report writing? Timeliness: Always report the incident as soon as possible. Accuracy: Make sure all information provided is accurate and detailed. The staff person involved or witnessing the incident completes the report and then it is submitted to a supervisor for review. If the staff person completing the report is a supervisor then a different supervisor should review and sign the report.
