To arrive at a basic price for a given service for a particular patient, each Medicare patient discharged by a PPS hospital is first assigned to a DRG that has a corresponding DRG weight. The DRG weight is multiplied by the hospitals payment rate per case.
The most commonly used payment systems to remunerate healthcare providers are salary, capitation, fee‐for‐service, pay for performance, and mixed or blended systems of payment. Salary: healthcare providers are paid based on the time spent at work.
Each hospital also has a blended base rate, which is based on a variety of factors, including location, patient demographics, whether its a teaching hospital, etc. The relative weight of the DRG is multiplied by the hospitals base rate to determine how much the hospital will be paid for that patient.
For example, a hospital in New York City pays higher wages than a hospital in rural Kansas, which is reflected in the payment rate each hospital gets for the same DRG.
