Hi everyone, Kevin here. Today I want to show you how you can create your very own fillable form using Microsoft Word. First off, what is a fillable form? You can have people go in and fill out a form, but they wont be able to edit the questions or the structure of the form. You can use things like check boxes, text fields, drop down lists, date pickers, to build your form. Once someones finished completing your form, they can e-mail it back, they could print it out, or you could even connect it to a database. Connecting to a database is outside of the scope of todays tutorial. Its kind of like an docHub PDF form, but its Microsofts version of it. Ive included sample files today if you want to follow along, otherwise lets jump on the PC and lets get started. Here I am in Microsoft Word and to be able to do this first you need a Word document that you want to transform into a form. Here I have an order form for the Kevin Cookie Company. Retired Soldiers who do not want to change their SBP elections to former spouse, either voluntarily or court ordered, must submit a DD Form 2656-6 (Survivor Benefit Plan Election Change Certificate) to DFAS with a copy of the divorce decree.
While on active duty, you are automatically enrolled in SBP at no cost to you until your date of retirement, at which time you must decide whether to continue SBP coverage and start paying a portion of the SBP cost.
The DD Form 2656-7 is used to establish the Survivor Benefit Plan after the passing of an active, reserve, or retired military service member. This checklist is designed for use by the spouse (widow or widower), eligible former spouse, or child of a deceased military member/retiree.
To convert your SBP election from spouse to former-spouse coverage you must submit a DD Form 2656-1, Survivor Benefit Plan (SBP) Election Statement for Former Spouse Coverage, to DFAS (Defense Finance and Accounting Service, US Military Retirement Pay, 8899 E 56th St, Indianapolis IN 36249-1200) within the first year
To change or update your SBP beneficiary designation, please complete a Survivor Benefit Plan Election Change Certificate (DD 2656-6). It is also available on our Forms webpage. Please only send copies of documents. Do not send the originals.
SBP-DIC Offset Elimination Beginning with the February 1, 2023 payday, the SBP-DIC Offset was fully eliminated. That means spouses now receive their full SBP monthly payments from DFAS and their full Dependency and Indemnity Compensation (DIC) payments from the VA.
There are limited conditions under which SBP may be re-started (or started) after retirement. Here are the common situations that apply. If spouse coverage is elected and the spouse is lost through death or divorce, SBP coverage is suspended.
The date of the spouses signature in item 32. b MUST NOT be before the date of the members signature in item 30. b, above.) The spouses signature MUST be docHubd.
