The students personal information is available for editing by the designated school official (DSO), when the student is in an Initial or Active status. DSOs are required to update this information in SEVIS within 21 days of the change.
Form I-983 Annual Evaluations The final evaluation on the lower half of page 5 is due by the end of your second year of STEM OPT. An additional final evaluation is due anytime you end a period of employment, no matter the duration of the position (use the evaluation range to indicate your employment end date).
Validation Reporting: A 6-, 12- and 18-month validation report to the DSO that confirms the students name, address, employer name and address, and employment status are correct.
You may apply for your OPT extension up to 90 days before your current OPT ends. You cannot apply once your 12-month OPT expires or during the 60-day grace period. Your complete OPT extension application must be received by USCIS before the expiration of your initial 12 months of OPT.
DSO Role with Form I-983 The student must submit the 12-month and final evaluations no later than 10 days following the conclusion of the appropriate reporting period. If an opportunity ends early the student must submit the final evaluation within 10 days following the conclusion of the opportunity.
Student must report their new worksite location by submitting a new Form I-983 and completed OPT Employment Reporting Form to ISSS within 10 days of beginning work at the new location.
