What is the difference between frequency code 6 and frequency code 7? Frequency code 6 is corrected claim and frequency code 7 is replace submitted claim. The difference is in how the payer handles it on their end.
Incomplete or invalid information is detected at the front-end of the contractors claim processing system. The claim is returned to the provider either electronically or in a hardcopy/checklist type form explaining the error(s) and how to correct the errors prior to resubmission.
The claim frequency codes are as follows: 1 Indicates the claim is an original claim 7 Indicates the new claim is a replacement or corrected claim the information present on this bill represents a complete replacement of the previously issued bill.
Common Re-Submission Codes Include: 6-Corrected. 7-Replacement. 8-Void.
NOTE: Box 9d on the HCFA / CMS 1500 form is where the Secondary Insurance for a patient populates.
7 = Replacement of prior claim. 8 = Void/cancel of prior claim.
Frequency code 8 Void/Cancel of Prior Claim: Indicates this bill is an exact duplicate of an incorrect bill previously submitted. This code will void the original submitted claims.
Use frequency code 7 to: Correct the date of service Correct patient data (except the subscriber ID; please submit a new day claim with claim frequency = 1 (CLM05-3).
