If the patient loses the ability to make MOLST decisions and the patient has already made decisions to withhold certain life-sustaining treatment (e.g. Do Not Resuscitate (DNR) and Do Not Intubate (DNI), the health care agent or surrogate cannot undo the patients decision.
MOLST is intended for patients with serious health conditions who: Want to avoid or receive any or all life-sustaining treatment; Reside in a long-term care facility or require long-term care services; and/or. Might die within the next year.
A healthcare proxy form is a legal document that is signed by you and two witnesses. It goes into effect when the patient who signed it is determined to lack the capacity to make his or her own healthcare decisions. A MOLST form is a medical order document, which is similar to a prescription.
The MOLST form is not an advance directive because it is a medical document that contains actionable medical orders that are effective immediately based on a patients current medical condition.
Here are some important areas an effective medical history form should cover: Patient contact information. Age and gender. History of surgeries and treatments. Previous tests and scans. Dates and timeline of symptoms. Family medical history. Past diseases and illnesses. Known allergies.
MOLST is an acronym for Medical Orders for Life Sustaining Treatment. These offer very specific options about what medical care you may or may not want CPR, ventilation, hospitalizations, intubation, artificial nutrition, hydration etc. It is signed by you and your physician. A HCP can sign a MOLST.
Patients who are appropriate for MOLST should have an up-to-date health care proxy in case MOLST orders need to be revised after the patient loses capacity. MOLST is not an Advance Directive.
The primary differences between the MOLST form and a DNR are: MOLST covers a variety of end-of-life treatments. A DNR only gives instructions about CPR. The MOLST form can also be used in a community setting where the DNR is intended to be used as a directive in a hospital setting.
