To replace a lost, stolen or damaged vehicle permit, visit a ServiceOntario centre. Bring the following with you: identification ( e.g. drivers licence, passport, Ontario Photo Card, etc. )
All driving licences issued by Driver Vehicle Agency (DVA) are now issued in a new format - a plastic photocard and a paper counterpart. Old format style licences are still valid and will be updated gradually as they are renewed and replaced.
You can renew a drivers licence, online or at a ServiceOntario centre, 180 days before it expires and up to one year after it expires.
If your EAD is lost, stolen or destroyed, you may request a replacement EAD by filing a new Form I-765 and filing fee (if required), unless a fee waiver is requested and approved. If you did not receive an EAD that USCIS mailed, you can submit an inquiry on non-delivery of a card.
To replace a lost, stolen or damaged Ontario Photo Card, you must complete the Ontario Photo Card application form available at a ServiceOntario centre and provide proof of: legal name. date of birth.
Theres nothing in the law that says you have 24 hours to produce it. Still, an officer might decide not to charge you, but you have to give your name and address to identify yourself. Police in Ontario can look it up on the spot to see whether you look like your drivers licence photo on file, Schmidt said.
If you are in Ontario, you will need to replace a lost, stolen or damaged drivers licence in-person at a ServiceOntario centre. A fee of $35.75 will be collected.
You must fill out an application and pay a fee to replace the permit.
You can download a copy of the drivers license application form from LTOs website and have it filled up. You will be requested to select your preferred LTO branch for the appointment. You may also opt to apply for a duplicate/replacement license and submit the affidavit of loss offline at any LTO office.
The Application for the driving licence form (DL1) is available at any driving licence testing centre or you can download it here. Having your form ready allows you to fall into the queue immediately. (Gauteng residents can apply online.)
