What is a hospital discharge letter? A hospital discharge letter is a brief medical summary of your hospital admission and the treatment you received whilst in hospital.It is usually written by one of the ward doctors.
Most discharge letters include a section that summarises the key information of the patients hospital stay in patient-friendly language, including investigation results, diagnoses, management and follow up. This is often given to the patient at discharge or posted out to the patients home.
You should be able to get a copy from the ward manager or the hospitals Patient Advice and Liaison Service (PALS).
A discharge summary will have been written by the doctor who was responsible for your care while you were in hospital. This is so your GP knows what tests and treatment youve had.
The patient will also be given a copy of this letter before they leave. Our AE aims to complete the discharge letter within four hours of the patients discharge. The letter will be sent electronically to your practice; however, if the patient does not remember their GPs details this letter will not be sent.
Dear Mr./Ms./Mrs.: I am writing to formally discharge you as a patient from my medical practice. Between now and (date one month from today), we will provide you emergency care as needed.
