If you think the information in your medical or billing record is incorrect, you can request a change, or amendment, to your record. The health care provider or health plan must respond to your request. If it created the information, it must amend inaccurate or incomplete information.
Texas law gives a deadline of 15 business days to provide medical records upon receipt of a request and any agreed upon fees. This same deadline also applies if the physician feels it would be harmful to release copies of medical records to a patient.
The Department of State Health Services is committed to providing full access to public information. To request records under the Texas Public Information Act: Submit a request in writing via U.S. Mail, fax or email. Include contact information and a clear description of the records you are requesting.
Your Medical Record number (MRN), is located on your After Visit Summary (AVS), or any medical record documentation from UMHS.
(a) An initial fee of $20.00 per request for a copy of the record. (b) Paper copies as follows: (i) One dollar per page for the first 20 pages. (ii) Fifty cents per page for pages 21 through 50.
Under the Michigan Public Health Code, medical records must be retained for a minimum period of seven years following the last date of service provided to a patient. However, this recommendation is a very general guideline that is subject to multiple modifications.
