hello my name is Jim Houser I am a social worker with pro health care and were going to be talking today about advanced directives and what you need to know about them the first part of this presentation is going to be talking in general about advanced directives more specifically a healthcare power of attorney and then as we go through well show you how to download the document and then the second part of the presentation is going to actually be going over the document and how to complete this this is a little bit of a strange situation because normally we would be doing this in person one-on-one or Ive done community presentations where we can respond to questions so without that we will be doing our best to cover as much as we can at the end of the presentation we will be providing some information on where you can call to get your questions answered so what I would suggest now if you dont have a paper-and-pencil the advantage of this electronic format is you can put this on pau

Respect for autonomy and fidelity to the patient are widely acknowledged as core values in the professional ethics of medicine. For patients who lack decision-making capacity, these values are fulfilled through third-party decision making and the use of advance directives.
They include patient, caregiver, legal, institutional, cultural, and religious factors, and while these factors may be similar between two societies, they do not have the same prevalence or contribute equally in each society.
As long as you are clear and thorough within your Advance Directive, there is little room for your desires to not be met. A disadvantage of an Advance Directive is that it can only account for the medical decisions that you have written down within the legal document.
Basic barriers include thinking that an advance directive isnt needed, not wanting to think about death or serious illness, not wanting to burden people, not knowing enough about advance directives and health care choices, needing help to fill out the forms, and lack of time with the doctor to discuss the matter.
Advance directives have limitations. For example, an older adult may not fully understand treatment options or recognize the consequences of certain choices in the future. Sometimes, people change their minds after expressing advance directives and forget to inform others.
The person should give a copy of the Advance Directive to her Agent if the she has one. The Agents job is to make sure that the persons decisions are known and followed. To do this, the Agent must have a copy of the Advance Directive that appoints her as the persons Agent.
The living will is too vague, unclear, or incomplete about what procedures or treatment options are allowed in specific circumstances. Medical technology has advanced since the living will was created and it is unclear whether the patient would wish to allow new treatments.
What to Include in Your Advance Directive. The name and contact information of your healthcare agent or proxy. Answers to specific questions about your preferences for care if you become unable to speak for yourself. The forms and questions asked vary a bit from state to state.
