A Direct Contracting Entity model framework is the performing entity comprised of strategic healthcare providers and suppliers, referred to as Participating and Preferred Providers, that operate in the program under a common legal structure.
Shared Savings Program Final Rule June 9, 2015 The final rule improves several program areas including: Creates a new performance-based risk option (Track 3) that includes prospective beneficiary assignment, a higher sharing rate, and the opportunity to use new care coordination tools.
DCEs negotiate capitated (per-patient) health care costs directly with CMS, sharing the risk that those costs will be higher than expected, resulting in a loss. They also share the benefit if the capitated cost turns out lower than expected.
Key Points. Medicare Direct Contracting Entities is a new way for CMS to pay for covered healthcare costs for those enrolled in Original Medicare. When you join a DCE, you are still enrolled in Original Medicare and can see any provider who accepts Medicare.
o Final Rule: The Final Rule allows low-revenue ACOs without prior ACO experience three years on the BASIC track under an upside-only agreement. Through incremental financial risk taking, low-revenue ACOs with prior ACO experience will remain on the BASIC track, while ultimately working to qualify as an Advanced APM.
Who can participate in the Direct Contracting model options? A key aspect of Direct Contracting is providing new opportunities for a variety of different organizations (Direct Contracting Entities or DCEs) to participate in value-based care arrangements in Medicare FFS.
Direct Contracting Entities (DCE) form relationships with two types of providers and/or suppliers: Participant and Preferred Providers. There are two key differences between these relationships. First, beneficiaries can only align to Participant Providers, not Preferred Providers.
ACOs are rewarded or penalized based on the service and performance they provide. They are paid or penalized depending on the total cost for a given payment year while DCEs come to terms with the Centers for Medicare Medicaid Services (CMS) for an agreed-upon monthly payment.
