What Is a Revision Laminectomy? In essence, a revision laminectomy is the same procedure as a standard laminectomy. It is simply being performed for a second time. The most common cause of this is the regrowth of bone spurs.
The relevant additional level codes are 63035 for initial discectomy and 63044 for the lumbar revision discectomy.
CPT 63042 is intended for a re-do discectomy and would be inappropriate to use for a re-do laminectomy. Rather, use 63047 for this service and modifier 22 may be appended if docHub additional work is documented.
If decompression is performed for stenosis/spondylosis via laminectomy WITHOUT facetectomy/Foraminotomy then you code 63005. If facecectomy/Foraminotomy documented then you code 63047,63048.
CPT code 63047 is a unilateral/bilateral code and is reported one time per lumbar level. The surgery at L5-S1 is reported as 63030-59 to indicate a distinct procedure was performed at a different level and is linked to a disk diagnosis.
