The most common method is to maneuver a drug-coated stent wrapped around a balloon into the middle of the closed-up stent. Inflating the balloon pushes aside the material obstructing the old stent and opens the new one. This often, but not always, solves the problem for good.
People who undergo angioplasty and stenting to treat coronary artery disease may develop in-stent restenosis. Scar tissue forms under the stent, causing a previously opened coronary artery to narrow again. You need another angioplasty or heart bypass surgery to correct the problem.
Restenosis is a gradual re-narrowing of the stented segment that occurs mostly between 3 to 12 months after stent placement. It usually presents as recurrent angina, but can present as acute myocardial infarction in approximately 10 percent of patients.
Normal Post-Stent Placement Symptoms Typically, the chest pain is caused by the expansion of the coronary artery during stenting and the discomfort is short-lived. However, patients may also experience muscular or superficial chest pain. This results from the physical stress of the procedure on the body.
The type of implanted stent, the presence of diabetes mellitus, previous bypass surgery, and small vessel caliber are predictors for ISR.
Biological factors associated with an increased risk of restenosis include diabetes mellitus and renal insufficiency. In a multivariate regression analysis, diabetes mellitus in itself increases the risk of BMS-ISR by 30% to 50%.
Its not uncommon for some scar tissue to form underneath this lining. But when theres too much scar tissue, the lining of the artery can get too thick and slow blood flow again. In-stent restenosis typically occurs within three to six months after treatment.
How Is Restenosis Treated? Medicine. If youve had restenosis more than twice in the same area, your doctor may prescribe sirolimus or cilostazol. Coronary artery bypass graft surgery. Percutaneous technique. Retrograde approach.
