Anyone can read and rate Community Notes, but only contributors whove unlocked the ability to write can add new notes to posts. Heres how to add a note: Click or tap the menu on the posts top right and then Write a Community Note.
Top ten tips for writing notes Date your notes and make the main topic visible. Dont write everything down write down the important points. Make short notes of the examples given. Use colour. Use illustrations and drawing. Use headings and sub-headings. Keep your sentences short.
Description of the issue/process/problem being documented, including (if applicable) how, when, and by whom the issue was identified, cause of issue (if known), corrective and preventive actions taken (when and by whom).
A Note to File is considered source documentation and must be signed and dated by either the person making the entry or the person reviewing and/or validating information the document contains. NTFs can be retained with the essential regulatory documents or in a subjects research chart as appropriate.
A Note to File (NTF), also known as a File Note, is written to document a discrepancy or other issue in the conduct of a research study. The NTF also provides a forum for you to document the action taken to correct this. The NTF can apply to a single participant, multiple participants or to the study/trial as a whole.
Here are some suggestions for making linear notes more useful. Use loads of HEADINGS for main ideas and concepts. Use subheadings for points within those ideas. Stick to one point per line. Underline key words. You can use numbering to keep yourself organised. Use abbreviations - and dont worry about using full sentences.
A note to file should: Be generated on a case-by-case basis. Include the subject and protocol it refers to. Be signed and dated by the individual who is writing it. Be legible if handwritten. Explain clearly and specifically the reason for the error/omission/discrepancy or process/policy it aims to address.
