The first step in managing a line is to assign a consistent place in the line for each student. Lining up students in a structured way will prevent them from running and pushing their way to the door when its time to line up. Some teachers have their kids line up alphabetically or in number order.
Here are a few for you and your team to consider: Assign student a seat that it is distant from the classroom exit. Arrange furniture / desk formations so that students exit route is partially obstructed. Place a child safety cover over the classroom door knob.
The five steps involved are the Anticipatory Set, Introduction of New Material, Guided Practice, Independent Practice and Closure.
Include the following types of lines: dotted, wavy, straight, spiral, fat/thin, curvy, zig zag, vertical, horizontal, diagonal, parallel, perpendicular, and angle. Have art prints/postcards available for students to look at. Ask them to describe the types of lines that they see in the pictures.
Make it difficult for the student to leave the classroom. Make sure the classroom door is always closed and seat the student as far away from the door as possible. To prevent her from leaving, especially during unstructured activities, position yourself near the door as much as possible.
To get my students to walk quietly in the hall I choose a line leader (at the beginning of the year its me). The students have to follow what the line leader does (similar to simon says without the talking). For example, we might pretend were birds and flap our arms and then tap our shoulders.

Encourage good behavior through chants. Make it a friendly competition. Mix up the order. Assign numbers. Try a classroom song.
