Put it in Add help text (In Rtf Template-Text form field options-Add help text). no need to select or change anything in query and format. Here use your field for which you want to change date format, Instead of Creation Dt. No need to change any formats in query and template
Date and time is collected in SDTM as character values using the extended ISO 8601 format. For example, 2019-10-9T13:42:00 .

The international standard recommends writing the date as year, then month, then the day: YYYY-MM-DD. So if both Australians and Americans used this, they would both write the date as 2019-02-03. Writing the date this way avoids confusion by placing the year first.
The format we are trying is DD-Mon-YYYY example: 01-Jan-2022.

If you want to use a date format ing to how another language displays dates, choose the language in Locale (location).
The United States is one of the few countries that use mm-dd-yyyy as their date formatwhich is very very unique! The day is written first and the year last in most countries (dd-mm-yyyy) and some nations, such as Iran, Korea, and China, write the year first and the day last (yyyy-mm-dd).
