383.73 and New Jersey regulations N.J.A.C. 13:21-23.28(l) require all Commercial Driver License (CDL) holders to submit a CDL Holder Self-Certification form ( view sample) and a valid (non-expired) Medical Examiner Certificate (MEC), if required.
All New York CDL holders are required to be medically qualified to drive a CMV by the means of a valid USDOT Medical Examiners Certificate (MCSA-5876), unless the type of commerce you drive qualifies as Excepted.
Intrastate Non-Excepted You must meet the Federal DOT medical certificate requirements. New Jersey regulations require a medical certificate for intrastate drivers.

A DOT physical exam is valid for up to 24 months. The medical examiner may also issue a medical examiners certificate for less than 24 months when it is desirable to monitor a condition, such as high blood pressure.
