Printing a one-off FP10 for a patient Go to the Prescription Search Screen and select the ETP Signing tab. Select the prescription from the list, right click on it and select Revert to FP10 option see below. Confirm the pop-up box to say that the prescription will be printed by FP10.
Prescribers must order private prescription pads from their Integrated Care Board (ICB). Community based prescribers (e.g. doctors, nurses or optometrists etc) are either working for a Sub ICB Location (SICBL) or for a Provider Organisation.
The 28 day period of validity runs from the date the prescription was signed unless the prescriber has specified a start date on the prescription. Schedule 2 and 3 CDs cannot be prescribed on repeat dispensing prescriptions.
A prescription, often abbreviated ℞ or Rx, is a formal communication from a physician or other registered health-care professional to a pharmacist, authorizing them to dispense a specific prescription drug for a specific patient.
All prescriptions must: State the name and address of the patient. Be written or printed legibly in ink. Be signed in indelible ink. Have an appropriate date (usually the date of signing) State the address of the prescriber. State the age of a child under 12.
Prescribing for patients in community settings will often occur on FP10 prescription forms which can be taken by the patient to a community pharmacist for dispensing.
NHS PRIMARY CARE PRESCRIPTION (FP10) 60 years of age or over or under 16 years of age. 16, 17 or 18 years old and in full-time education. Maternity exemption certificate expectant mothers and those who have given birth to a stillborn child in the last 12 months. Medical exemption certificate.
Instructions. 1) This form replaces the Letter of Medical Necessity. Use this form to be reimbursed for products and services that require physician authorization such as Over-the-Counter (OTC) medicines or drugs and other non-OTC medicine products and services.
