Most of the form can be filled out prior to visiting the affidavit taker, but do not sign it beforehand; this must be done in their presence. A list of authorized affidavit takers is available at justice.vic.gov.au. During the signing, you must initial any changes, sign each page, and the affidavit taker will also sign and initial any alterations. If the affidavit references additional documents, the affidavit taker must attach a certificate identifying those documents.
