The three most accurate tests for detecting an ACL tear are the Lachman test (sensitivity of 60 to 100 percent; mean 84 percent), the anterior drawer test (sensitivity of 9 to 93 percent; mean 62 percent), and the pivot shift test (sensitivity of 27 to 95 percent; mean 62 percent).
There are four commonly used grafts for ACL reconstruction. Patellar tendon autograft. The surgeon removes the middle third of your patellar tendon and pieces of bone (bone plugs) from your shin and kneecap. Hamstring tendon autograft. Quadriceps tendon autograft. Allograft.
The anterior Lachman test, anterior drawer test and the pivot shift test, which are summarised in Table 1, are the most commonly known physical tests used to assess the integrity of the ACL (Benjaminse 2006; Leblanc 2015; Malanga 2003; Scholten 2003; Solomon 2001).
Signs of ACL graft failure? The signs of ACL graft failure can include swelling, pain within the knee, locking within the knee, a mechanical block (which can be due to a bucket-handle tear of the meniscus), lack of full motion, and difficulty with twisting, turning, and pivoting.
The Lachman test is the most sensitive and the pivot shift the most specific test for the diagnosis of ACL rupture.
The most commonly used grafts for ACLR include hamstring tendon autografts, bonepatellar tendonbone autografts, quadriceps tendon autografts, allografts and synthetic grafts.
ACL LAXITY TESTS The most frequently used tests for assessing ACL laxity are the anterior drawer test, Lachman test, Maclntosh test, jerk test, flexion rotation drawer test, Slocum test, and Losee test.
A positive Lachman test or pivot test is strong evidence of an existing anterior cruciate ligament (ACL) tear, and a negative Lachman test is fairly good evidence against that injury. Although widely used, the anterior drawer is the least helpful maneuver for diagnosing an ACL tear.
