How is rehabilitation after an ACL reconstruction typically structured? Phase I - First two weeks after surgery. Phase II - two to six weeks after surgery. Phase III - six weeks to three to four months after surgery. Phase IV - four to six months after surgery. Phase V - return to sport, usually at six months.
How will I know if I retear my ACL? Rapid and large amounts of swelling. A popping sound or feeling. Lots of pain. Inability to continue playing the sport.
In some cases, an ACL tear can appear similar to ACL degeneration, which is a more long-standing process. A partial tear of the ACL may also be misdiagnosed as a normal ACL since these injuries can be subtle in appearance. ACL tears are also often misdiagnosed as collateral ligament knee strains.
Over 50% of ACL tears can heal by themselves with no focused treatment. Research has found that 56% of tears spontaneously heal at 2 years and 58% at 5 years. Instead of rushing to surgery, the focus should be on the self-healing potential of the ACL! A fracture requires 6-8 weeks to heal.
The highest level of evidence for ACL rehabilitation in this review (moderate certainty) was observed for: (1) neuromuscular electrical stimulation to improve quadriceps strength; (2) open versus closed kinetic chain exercises to be similarly effective for improving quadriceps strength and self-reported function; (3)
Some other things that could mimic an ACL injury could be a fracture (broken bone), dislocation, meniscus tear, cartilage injury, or another ligament injury. If this type of injury occurs, you should seek immediate medical attention for appropriate diagnosis and treatment.
But each year, there are an estimated 200,000 ACL ruptures in the US alone, making it the most commonly injured knee ligament. Typically, a diagnosis is made after a clinical exam and a knee MRI. However, its common for physicians unfamiliar with ACL injuries to misdiagnose them.
Tibial spine fractures are ACL equivalent injuries, and should be operatively managed if displaced.
No pain or swelling, full range of motion in the knee, and good muscle strength and endurance are all signs your recovery is going smoothly.
Although the symptoms are similar, a few key differences will help you identify whether the injury affected the ACL or MCL. An ACL tear will have a more distinctive and loud popping sound than an MCL tear. Additionally, the location of your pain and swelling could indicate either an ACL or MCL tear.
