In general, there are three main graft options: a patellar tendon autograft, a hamstring autograft, and an allograft. An autograft comes from the patients own body, whereas an allograft comes from a cadaver donor.
The ACL functions to prevent posterior translation of the femur on the tibia (or anterior displacement of the tibia) during flexion-extension of the knee.
The ACL is a crucial element in stabilizing the knee joint. It primarily stabilizes the anterior tibial translation and plays a small role in resisting internal rotation and prevents any excessive movements.
The posterior cruciate ligament (PCL) is one of two cruciate ligaments of the knee, which is the counterpart to the anterior cruciate ligament (ACL). The PCL is an important restraint of posterior tibial translation relative to the femur.
The most common cause of ACL failure is a technical error in the first surgery. The two most common technical errors are placing the ACL reconstruction graft on the femur too anterior (away from the back wall) or placing the tibial tunnel too posterior (posterior to the anterior horn of the lateral meniscus).
The PCL prevents the tibia from moving backwards from the femur (posterior translation) and the ACL prevents the tibia from moving forward (anterior translation). Both ligaments help provide rotational stability to the joint.
Specifically, the ACL (anterior cruciate ligament) prevents excessive anterior translation of the tibia on the femur, as well as abnormal rotation.
Its important to remember that, during the first 4-12 weeks after your surgery, your ACL graft is at its weakest point in the recovery process, since the graft requires time to grow and adapt to the bone and tendon.
