Does a Medical Power of Attorney need to be docHubd, witnessed, and/or recorded in Virginia? The specific requirements vary in each state; however, in Virginia, your document will typically need to be signed by two witnesses.

Does an Advance Directive in Virginia need to be docHubd? No. An Advance Directive registry for Virginia is being developed, and when it becomes operational, a notary will be required to post an Advance Directive on the registry. Advance Directives without a notary still will be valid for other purposes.
Do I need to have my advance directive docHubd (legally making a document valid)? No. In the state of Pennsylvania, you do not need to docHub advance directives.
Any California resident who is at least 18 years of age and mentally competent may execute a valid AHCD. Two qualified adult witnesses or a notary public must sign the AHCD, acknowledging that you are competent and acting under your own volition. No attorney is required.
Witness means any person over the age of 18, including a spouse or blood relative of the declarant. Employees of health care facilities and physicians offices, who act in good faith, shall be permitted to serve as witnesses for purposes of this article. 1983, c. 532, 54-325.8:2; 1984, c.
The law specifically states that a Directive to Physicians does not have to be docHubd. A physician, health care facility, or health care professional may not require it be docHubd nor may any of them require you to use a specific form.
All adults in Virginia have a right to prepare a document to put their medical care wishes in writing. An Advance Directive lets other people know the types of medical care you do and do not want if you are unable to express your wishes on your own. Many people call these instructions a living will.
