Normal AFP levels are present in as many as 30% of patients at time of diagnosis and usually remain low, even with advanced HCC 39. AFP 400500 ng/ml is considered diagnostic for HCC, although fewer than half of patients may generate levels that high 39.
Conclusion. AFP levels in serum showed good accuracy in HCC diagnosis, and the threshold of AFP with 400 ng/mL was better than that of 200 ng/mL in terms of sensitivity and specificity no matter AFP is used alone or combined with ultrasound.
Alpha-fetoprotein (AFP) is the most widely used serum marker for screening and initial diagnosis of HCC in clinical practice. However, the sensitivity of AFP is only about 60% at a cut-off value of 20 ng/mL, and the specificity is low [8,9,10].
AFP, as a result, is frequently adopted as a diagnostic tool for HCC in high risk patients. However, AFP levels below 100 ng/mL are less specific since slightly elevated AFP can also be observed in patients with chronic hepatitis [4]. Moreover, studies have shown that about 40% of HCC had normal AFP levels [4,8,9].
Alpha-fetoprotein (AFP) is the most widely used biomarker for HCC diagnosis. Based on elevated serum AFP levels as well as typical imaging features, AFP-positive HCC (APHC) can be easily diagnosed, but AFP-negative HCC (ANHC) is not easily detected due to lack of ideal biomarkers and thus mainly reliance on imaging.
In healthy adults, serum AFP level typically falls into the range of 510 ng/mL [7]. On the other hand, an elevated serum level of AFP is frequently associated with HCC or other liver diseases. Studies have shown that an AFP level above 400 ng/mL can generally be considered as diagnostic for HCC [3].
The AFP concentration in maternal serum rises throughout pregnancy, from the nonpregnancy level of 0.2 ng/mL to about 250 ng/mL at 32 weeks gestation.
With combined use of concanavalin A (Con A) and lentil agglutinin (LCH) binding tests, AFPs were classified into three subtypes: benign hepatic condition type (six patients), hepatocellular carcinoma type (nine patients) and yolk sac type (12 patients).
