If you test positive for HPV 16/18, you will need to have a colposcopy. If you test positive for HPV, but did not have genotyping performed or had genotyping but tested negative for 16/18, you will probably need to have a colposcopy.
Around 14 types of HPV are considered high risk for cervical cancer. Two of these types (HPV 16 and HPV 18) cause about 70 out of 100 (70%) of all cervical cancer cases. In most people, the immune system clears about half of HPV infections within 6 to 12 months.
Sometimes, after several negative HPV tests, a woman may have a positive HPV test result. This is not necessarily a sign of a new HPV infection. Sometimes an HPV infection can become active again after many years. Some other viruses behave this way.
HPV 16 is the highest-risk HPV type. Expedited treatment should be considered for HSIL cytology results, and colposcopy is recommended in all other cases, even if the cytology test is normal.
An abnormal Pap test result rarely means cancer is present, especially in someone who has regular, annual Pap tests. An abnormal result means there are cells in the cervix that look unusual, which can be caused by yeast or bacterial infection, irritation, benign growths, hormonal changes, or pre-cancer.
Certain things may interfere with a Pap test including: Menstruation. Using products such as vaginal creams, jellies, medicines, or spermicidal foams for 2 to 3 days before the Pap test. Douching for 2 to 3 days before a Pap test as douching can wash away surface cells.
Women with a positive oncogenic HPV (not 16/18) test result, with a LBC prediction of pHSIL/HSIL or any glandular abnormality, should be referred for colposcopic assessment at the earliest opportunity, ideally within 8 weeks.
