If the donor can provide a valid prescription (which the MRO verifies with the pharmacy) or other reason a drug test result may be positive, then the MRO may report a negative result. If the MRO is not able to contact the donor within a specific amount of time, then the result is reported as a Non-Contact Positive.
If none is given, a good model to follow, even if you are not mandated to, is the Department of Transportations (DOT) guidelines for handling a negative dilute result. DOT gives employers the option of either treating the result as a negative or requiring that the employee submit another urine specimen.
If you are seeing results that show you are getting close to a dilute sample, try to stop drinking any fluids 2-4 hours prior to providing the sample. Then resume your normal fluid intake. You should not hold your urine for a long time before giving a sample.
Invalid result: Refers to the result reported by a laboratory for a urine specimen that contains an unidentified adulterant, contains an unidentified interfering substance, has an abnormal physical characteristic, or has an endogenous substance at an abnormal concentration that prevents the laboratory from completing
Non-negative results require a sample to be sent for confirmation (GCMS/LCMS) testing in a NATA accredited laboratory. The confirmatory testing can determine drug concentration levels and if the result may be consistent with medications, foodstuffs or illicit drug use.
If the laboratory testing comes back and finds that the consumption of the substance (or substances) is below the limits set by the European Workplace Drug Testing Society, then the non-negative result will be converted into a negative result or will show a high consumption level above these limits.
Drugs/Drug classes reported as Positive are automatically reflexed to mass spectrometry confirmation/quantitation. An unconfirmed positive immunoassay screen result may be useful for medical purposes but does not meet forensic standards.
A CDL driver who tests positive on a DOT test or refuses to take a DOT test, must successfully complete the return-to-duty (RTD) process with a DOT qualified substance abuse professional (SAP). The requirements for the RTD process are found in 49 CFR Part 40, Subpart O. See 49 CFR 382.501, 382.503.
