This review by Scholten et al suggests that during the physical examination of the knee, a negative Lachman test would indicate no anterior cruciate ligament (ACL) tear, whereas a positive pivot shift test would indicate an ACL tear.
The Lachman test is a specific clinical exam technique used to evaluate patients with a suspected anterior cruciate ligament (ACL) injury. The test relies on proper positioning and technique and is regarded as the most sensitive and specific test for diagnosing acute ACL injuries.
Physical and Occupational Therapy Riding a stationary bike may help to reduce swelling, and stretching the knee, thigh, and lower leg can help improve range of motion. After swelling has subsided, your physical therapist may concentrate on strengthening the muscles around the knee to help support it.
A positive Lachman test or pivot test is strong evidence of an existing anterior cruciate ligament (ACL) tear, and a negative Lachman test is fairly good evidence against that injury. Although widely used, the anterior drawer is the least helpful maneuver for diagnosing an ACL tear.
Anterior translation of the tibia associated with a soft or a mushy end-feel indicates a positive test. More than about 2mm of anterior translation compared to the uninvolved knee suggests a torn ACL (soft end-feel), as does 10mm of total anterior translation.
The prone Lachman test has shown higher accuracy than the pooled sensitivity (52%58%) and specificity (90%94%) values for the anterior drawer test published by Benjaminse et al.
Contralateral means on the other side. For ACL injuries, it refers to the currently uninjured knee.
Post Cruciate Ligament (PCL) The posterior cruciate ligament (PCL) is located inside the knee joint and runs just behind the ACL. The PCL prevents backward (posterior) translation of the tibia (shin bone) with respect to the femur (thigh bone).
