Recent national consensus guidelines recommend an optimal margin width of 2 mm or greater for the management of DCIS; however, controversy regarding re-excision remains when managing negative margins 2 mm.
Patients who have an invasive cancer with an intra-ductal component should be treated based on the invasive cancer recommendations. Specifically, a margin of less than 2mm for the DCIS component in a specimen also containing invasive cancer is acceptable.
Lumpectomy margins are the rim of normal tissue surrounding the cancer tumor that is often removed with the tumor during the surgery to ensure the cancer is completely gone. After the procedure, a pathologist examines the removed tissue to check for remaining cancer cells in the lumpectomy margins.
A lumpectomy with clear margins may be the only surgery needed to treat breast cancer. Positive margins, on the other hand, may indicate that all of the cancer was not removed, and another surgery in the same area may be needed to remove more tissue.
A positive surgical margin at prostatectomy is defined as tumor cells touching the inked edge of the specimen. This finding is reported in 8.8% to 42% of cases (median about 20%) in various studies.
(MAR-jin) The edge or border of the tissue removed in cancer surgery. The margin is described as negative or clean when the pathologist finds no cancer cells at the edge of the tissue, suggesting that all of the cancer has been removed.
The invasive cancer margin guideline endorses no ink on tumor, while the DCIS guideline states that 2mm is an optimal margin. This raises the question of which guideline to apply in microinvasive carcinoma, or when DCIS occurs in association with invasive carcinoma and the DCIS component is in proximity to the margin.
A cutoff of less than 1 mm between cut tissue edge and invasive tumor would be appropriate to consider as a close margin at risk for recurrence.
