The anterior Lachman test, anterior drawer test and the pivot shift test, which are summarised in Table 1, are the most commonly known physical tests used to assess the integrity of the ACL (Benjaminse 2006; Leblanc 2015; Malanga 2003; Scholten 2003; Solomon 2001).
The ACL can be injured or torn in a number of different ways. The most common mechanism is that of a sudden pivoting or cutting maneuver during sporting activity, which is commonly seen in football, basketball and soccer. The ligament can also tear due to work injuries or automobile accidents.
This explains why most ACL tears occur without contact. The PCL, on the other hand, does the opposite, preventing the tibia from sliding backwards relative to the femur.
Exam Maneuvers Lachman Test: this is the most sensitive physical exam maneuver for detecting a torn ACL. It is performed by stabilizing the femur while the tibia is pulled forward (see Figure 1). A positive test is signified by excessive forward translation without a firm endpoint, indicating disruption to the ACL.
Symptoms A loud pop or a popping sensation in the knee. Severe pain and inability to continue activity. Rapid swelling. Loss of range of motion. A feeling of instability or giving way with weight bearing.
The cruciate ligaments control the front and back motion of your knee. The anterior cruciate ligament runs diagonally in the middle of the knee. It prevents the tibia from sliding out in front of the femur and provides rotational stability to the knee. The PCL keeps the shinbone from moving backward too far.
Types of ACL Surgery Autograft. Your doctor uses a tendon from somewhere else in your body (like your other knee, hamstring, or thigh). Allograft. This type of graft uses tissue from someone else (a deceased donor). Synthetic graft. This is when artificial materials replace the tendon.
The Lachman test is a specific clinical exam technique used to evaluate patients with a suspected anterior cruciate ligament (ACL) injury. The test relies on proper positioning and technique and is regarded as the most sensitive and specific test for diagnosing acute ACL injuries.
