I appoint this person to make decisions about my medical care if there ever comes a time when I cannot make those decisions myself. I want the person I have appointed, my doctors, my family and others to be guided by the decisions I have made in the parts of the form that follow.
The Health Care Proxy Law lets you appoint a competent adult to make decisions about your medical treatment in the event you lose the ability to decide for yourself including decisions to remove or provide life-sustaining treatment. You can appoint a family member, a close friend, or anyone you choose.
You do not need to have the form docHubd. The standard Health Care Proxy form approved under New York law has sections that must be completed for your proxy to be valid. The form also has optional sections that you may or may not choose to complete.
These include: Living Will. A living will is a written document that specifies what medical treatment you would or would not want in the event you are in a terminal condition or a persistent vegetative state. Power of Attorney. Health Care Instructions.
Can you appoint more than one person to act as agent at the same time? No. The New York State Department of Health has stated that each person can appoint only one agent.
What to Include in Your Advance Directive. The name and contact information of your healthcare agent or proxy. Answers to specific questions about your preferences for care if you become unable to speak for yourself. The forms and questions asked vary a bit from state to state.
Normally, one person (not multiple persons to act at one time) is appointed as your health care proxy. It is quite common, however, for you to appoint one or more alternate persons (successors) in the event your first choice proxy is unavailable.
If I name a health care proxy, I give up the right to make my own decisions. False. Naming a health care agent proxy does not take away any of your authority. You always have the right, while you are still competent, to override the decision of your proxy or revoke the directive.
