This will bring up the Peralta Campus Solutions login page. Sign in by entering your user ID, then enter your password. Next, click Sign In at the bottom of the screen. The main Campus Solutions page for students will open up. To add or make changes to your personal information, find the box named Profile and click on it. The Personal Details window will open up. To add or edit a preferred first name, click on the second box, which is located under the Primary Personal Details box. The Edit Name window will open. In the First Name field, type in your preferred first name. Then, locate the Save box on the upper, right-hand corner and click on it. In order to obtain a Social Security Number (SSN) and card, the Social Security Administration (SSA) requires F-1 students to provide proof of valid F-1 status, proof of employment and of work authorization. UNT Department Employment Letter (From the Department you work for on campus.)
To request a different Social Security number, contact your local Social Security office for an in-person appointment.We can assign a different number only if: Sequential numbers assigned to members of the same family are causing problems. More than one person is assigned or using the same number.
How do I correct or change my official/legal name at the University? For students, changes to name, Social Security number, date of birth, citizenship, ethnicity, or gender may be processed by submitting a Change of Personal Information form to the Office of Registration and Transcripts.
To update your SSN or Student ID Number please utilize the Change or Correct Records forms. If submitting in person, the Registrars Office must make copies of the original legal documents before submitting the official change to the University. Documents submitted via secure upload must be docHubd.
Please complete a Name Change Form and provide the legal document, i.e. drivers license, marriage certificate, social security card, etc., reflecting your new name. If you are an employee (faculty, staff, or student), you must also contact the Human Resources Office regarding a name change.
Change Your Name UNT will use your primary (legal) name on your transcript, diploma, and on some pages in the Student Center within MyUNT. To update any part of your primary name, you must present the Registrars Office with 2 forms of legal documentation substantiating the name change.
Complete Form SS-5. You can download this online or get one in any SSA offices. Mail or submit your documents in person. Though updating your records in person is much faster than doing it by mail.
